- Eligibility: Adults with disabilities over the age of 60 who are town residents and meet the required standards of independence., Age 60 Plus, Boston HealthNet Recipients Only, Cambridge Health Alliance clients, Cancer patients only, Disabled persons are required to fill out a form, Must be 60 years of age or older and low-to- moderate income resident, Must be 60 years or older in participating communities ONLY, Must be 60 years or older or adult with disabilities in participating communities ONLY, Must be 65 years or older or adult with disabilities in participating communities ONLY, Must be a senior 55 years of age or older., Requires ADA application approval, Rider must have emergency contact information listed with the COA office, Rider states they are eligible, There is no application process for Seniors, Unable to get to medical appointments independently, but able to get in and out of car unassisted.
