Description: The following services are provided for RESIDENTS OF CHATHAM only.  
HOURS OF OPERATION: CCOA’s business hours are Monday-Friday, 8am-4pm.  Holiday transportation is not available.  If the Monomoy School System and/or Chatham Town Hall are closed due to inclement weather, CCOA is closed, and transportation will not be available. Transportation may also be cancelled if the CCOA Drivers determine that vehicle operating conditions are too hazardous.

SCHEDULING: By reservation only, call CCOA at 508-945-5190. First come, first served. Most rides occur Monday-Friday, between 9am and 2pm. En route stop requests will be handled according to schedule demands and the CCOA Driver’s discretion.
AREAS OF SERVICE: Chatham, Orleans, Dennis and Harwich, subject to schedule and driver availability.
TRANSPORTATION PRIORITIES & SERVICE: 
– Medical appointments will receive priority scheduling
– Medical rides available between Orleans and Hyannis
– Rides to Bank, Post Office, hairdresser, etc. must be scheduled for Chatham, Harwich, or Orleans
-Rides to CFAL programs or appointments available.
-Weekly Grocery and RX delivery (Monday & Thursday): 24 hours advanced scheduling
-Weekly AM trip to Stop & Shop (Thursday): 24 hours advanced scheduling

COMMUNITY TRANSPORTATION:
Contact CCRTA (800-352-7155) for DART, Smart DART, H2O, FLEX, and Boston Hospital Transport services.

ASSISTANCE REQUIREMENTS: CCOA reserves the right to request that a personal caregiver or aide accompany a rider.  All riders must be able to enter and exit the vehicle without CCOA Driver assistance. CCOA Drivers are not permitted to attend medical appointments with clients, provisions for such attendance are the responsibility of the client, family or caregiver.
ELIGIBILITY:
CCOA transportation is a service offered to all Chatham residents aged 60 and over. All riders must complete a Transportation Assessment with a CCOA Outreach Coordinator before service will begin. All riders must be able to enter and exit the vehicle without assistance, unless accommodations are requested due to disability. Oxygen is allowed, per ADA requirements. Oxygen tanks must be secured and may not block any aisles. The COA must be advised of rider’s supplemental oxygen needs when making the reservation. Transportation will not be provided to anyone who has received any form of anesthesia. Transportation will not be provided to residents of skilled nursing facilities.
