- Eligibility: Adults with disabilities over the age of 60 who are town residents and meet the required standards of independence., Age 60 Plus, Boston HealthNet Recipients Only, Cambridge Health Alliance clients, Cancer patients only, Disabled persons are required to fill out a form, MGH patients, visitors, or employees only, Must be 60 years of age or older and low-to- moderate income resident, Must be 60 years or older in participating communities ONLY, Must be 60 years or older or adult with disabilities in participating communities ONLY, Must be 62 years of age or older, Must be a resident of the town, Must be a senior 55 years of age or older., Must have a Medical Referral, Persons w/ Disabilities, PT1 Required for MassHealth Transportation, US Veterans
